All marriage licenses cost $40. Please pay at the end of application process.

Important information concerning a Marriage License

* Note: If you do not have a social security number please enter 000000000 on the form *

When issuing a marriage license the New York State Department of Health requires the following acceptable forms of I.D. for current names:

Group A: Items A1 and A6 MUST HAVE A PICTURE and be current:
1. Driver’s License
2. Non-Driver’s License
3. Passport (valid)
4. Naturalization papers – only original papers are acceptable.
5. Military I.D.
6. Employees photo I.D.

Group B: Items B1 and B2 must be translated into English if in another language
1. Birth Certificate
2. Baptismal Records

Each partner must upload online and present in person one ID from Group A and one ID from Group B.
Only the name that appears on the documentation is acceptable. We cannot “Americanize” a name. The Bride and groom may not use their current name as a middle name. If both names are used they must be separated by a hyphen.

Only certifiable translators are acceptable.
